Section 2: The Missing Layer

From the perspective of my 27-year body of work and the patterns I’ve been tracking (readiness-first, upstream diagnostics, boomerang cycles), the above graphics—and the frameworks they aggregate—still start too late in the sequence. As a result, the initiative failure rate of 70% to 80% has persisted over several decades and, if unchanged, will continue to do so in the AI era.

Phase 0: Readiness Assessment — The gate before everything else. Everyone starts at the frameworks. Winners start at Phase 0.
